Adding Conditional Logic to Forms
Conditional logic for forms allows questions to dynamically adjust based on responses, improving efficiency, ease of use, and streamlining the questions that need to be answered.
Key Benefits
- Improves efficiency and ease of use by showing only relevant questions based on responses, reducing time spent completing forms.
- Enhances data quality and consistency by guiding users through the right questions and minimizing unnecessary or incomplete responses.
Steps to Add Conditional Logic
-
Navigate to the Inform or Script you would like to update. If the form was privately created, open the report and select the Manage Questions tab. If the form is organization‑level, please reach out to your health system administrator or Laudio Support for help making updates.
-
Locate the question you want to use as the trigger for conditional logic. Click the cascade icon located in the bottom‑right corner of that question.

-
A slide‑out panel will appear. In the field labeled “If answer equals,” select the response that should trigger additional questions. Then select one or more follow‑up questions that should appear when this condition is met.

-
Once saved, the conditional logic will visually appear on each question included in the logic flow. This visual indicator confirms how questions are connected within the form.
