InForms Overview

Laudio InForms are customizable and/or standardized questionnaires you can send to team members or collect feedback anonymously across a variety of use cases.

Where to Find InForms

While InForms are integrated throughout Laudio's platform, the dedicated workflow can be found on the left-hand side of the navigation bar.

 
Within the InForms workflow, forms that are standardized and created across your health system will automatically populate on the main InForms page. To add a Laudio or health system–specific template (which copies a form into your own Laudio instance) or to create a new, personal-use InForm, click "Add New InForm" at the top of the page.
 

FAQs

Q: If a leader sends an InForm via email and a team member responds, can the leader see their responses?
A: Yes. When a team member completes an InForm sent via email, the leader automatically receives an email notification with a link to review the response.

Q: If a leader adds a note to an InForm response submitted via email, will the team member see that note?
A: It depends. When adding a supplemental note, the leader can choose whether the team member can see it by clicking or unclicking the "eye" icon.
