InForms Overview

InForms are customizable and standardized questionnaires that help frontline leaders collect feedback from team members. InForms can be sent directly to team members, shared anonymously through public links or QR codes, and used across a variety of engagement, feedback, and rounding workflows.

Where to Find InForms

InForms are available throughout Laudio, but the dedicated InForms workflow can be accessed from the left-hand navigation menu.

Within the InForms workflow, standardized forms from across the health system automatically appear on the main InForms page. To use an existing template or create a new form, click Add New InForm. Leaders can add a Laudio or health system template to their own Laudio instance or create an InForm for personal use.

Q: If a leader sends an InForm via email and a team member responds, can the leader see their responses?
A: Yes. After a team member completes an InForm sent by email, the leader receives an email notification with a link to review the submitted response.

Q: If a leader adds a note to an InForm response, can the team member see it?
A: It depends. When adding a supplemental note, the leader can control visibility by selecting or clearing the Eye icon. If the icon is enabled, the team member can view the note. If it is disabled, the note remains private.
