Using the Patient Rounding Tracker

The Patient Rounding Tracker shows a real time list of patients for a selected unit and captures patient feedback as it is documented. It allows leaders to view patient information, identify trends, and take action when needed.

Using the Tracker

  1. Click Patient Rounding from the list of workflows in the left hand navigation bar.

  2. The Tracker will open automatically.

  3. Review the patient list displayed. The columns are sortable and include the following information: 

    • Patient name and current status

    • Room and bed information

    • Admit time and length of stay

    • Time of the last round

    • Last Round Score
  4. Additional filters in the top right of the tracker allow leaders to change the visible unit, and toggle between current and past patients.

  5. Use this view to sort patients, log rounds, review responses by patient, and prioritize follow ups from a single screen.

FAQs

Q: How often is the dashboard updated?
A: The dashboard updates in real time using ADT data from Epic.

Q: Can past patients be viewed on the tracker, and can rounds be conducted for past patients?
A: Yes. Past patients can be viewed and rounds can be documented. Past patients remain visible for a limited number of days, such as 30, based on your health system configuration.

Q: What do New Admit and Discharged statuses mean?
A: New Admit - The patient arrival occurred within the last 24 hours. Discharged - The patient has been discharged from care.

Q: What does Room and Bed show?
A: Room and Bed shows the patient’s current location.

Q: What does Length of Stay mean?
A: Length of Stay reflects the patient’s total time in the hospital, not time spent in the current unit. It is calculated from the hospital admission time through the discharge date and time. If a patient has multiple active visits, the most recent visit is used.

Q: What does Last Round show?
A: Last Round shows the date and time the most recent round was documented for the patient in the current unit.

Q: What does Last Score show?
A: Last Score shows the score from the most recent round completed in the current unit.
